Be Proactive about Your Care - Part 2

Proper care of patients with diabetes requires a comprehensive management plan. The American Diabetes Association recommends that you review the following items from your management plan with your doctor at your follow-up visits to determine progress in meeting your goals and to identify any problems:

  • Short-term and long-term goals
  • Medications
  • Glycemia
  • Frequency/severity of hypoglycemia
  • Results from self-monitoring of blood glucose
  • Complications
  • Control of high cholesterol levels
  • Blood pressure
  • Weight
  • Diet and nutrition
  • Exercise regimen
  • Any necessary referrals
  • Psychosocial adjustment
  • Knowledge of diabetes
  • Self-management skills
  • Smoking cessation, if indicated

Print this out and take it with you to your next appointment. Let your healthcare provider know that you are actively involved in self-management of your diabetes and expect to receive the recommended care.
